Gum Disease Is Very Common — and Very Treatable

Gum disease (periodontal disease) is one of the most prevalent oral health conditions among adults. It begins when plaque — a soft film of bacteria — accumulates along and below the gum line and is not fully removed through brushing and flossing. Over time, plaque hardens into tartar (calculus), which cannot be removed at home and requires professional care.

Left unaddressed, gum disease can progress from mild inflammation of the gums (gingivitis) to a deeper infection involving the bone and tissues that support your teeth (periodontitis). The encouraging news is that with appropriate professional care, many patients experience meaningful improvement in their gum health.

Understanding the Stages of Gum Disease

Stage 1

Gingivitis

The earliest stage — the gums are inflamed, may bleed when brushed, and may appear red or swollen. At this stage, the bone and connective tissue have not yet been affected. With proper professional cleaning and improved home care, gingivitis is reversible.

Stage 2

Mild Periodontitis

The infection has begun to affect the bone supporting the teeth. Pockets between the gum and tooth have deepened. A deep cleaning is typically recommended at this stage to remove the bacteria and tartar below the gum line that a routine cleaning cannot reach.

Stage 3

Moderate Periodontitis

Deeper pockets and more significant bone loss. Teeth may begin to feel sensitive. Deep cleaning may be performed in multiple sections, and closer monitoring at follow-up visits is typically recommended.

Stage 4

Severe Periodontitis

Significant bone involvement; teeth may feel loose. In some cases, a referral to a periodontist (gum specialist) is appropriate. Dr. Maddipati is straightforward about when a specialist referral would serve you best.

What Periodontal Deep Cleaning Involves

A periodontal deep cleaning — also called scaling and root planing (SRP) — is a non-surgical procedure that addresses the plaque, tartar, and bacteria that have accumulated below the gum line. It goes beyond what a routine cleaning can accomplish and is specifically designed to treat active gum disease.

Dr. Maddipati uses a small, graduated probe to gently measure the depth of the pockets around each tooth. She reviews these measurements with you — healthy pockets are shallow; deeper pockets with bleeding and tartar indicate disease activity that a routine cleaning cannot adequately address.

Local Anesthesia for Your Comfort

The area being treated is numbed with local anesthetic before the procedure begins. Many patients find the treatment more comfortable than they anticipated. Dr. Maddipati checks in with you throughout to ensure you feel at ease.

Scaling — Removing Buildup Below the Gum Line

Specialized instruments — including ultrasonic scalers and hand instruments — are used to carefully remove plaque and hardened tartar from the root surfaces and within the pockets. The goal is to create a clean surface to which the gum tissue can respond positively.

Root Planing — Smoothing the Root Surfaces

The root surfaces of the teeth are carefully smoothed to make it more difficult for bacteria to re-adhere and to support the gum tissue in healing. This step is what differentiates a deep cleaning from a standard scaling.

In some cases, antimicrobial agents may be applied to the treated areas to support healing. Dr. Maddipati will discuss this with you if it is appropriate for your situation.

A re-evaluation appointment — typically scheduled after the tissues have had time to respond — allows Dr. Maddipati to measure pocket depths again and assess how the gums have responded. This helps determine whether additional treatment or a closer maintenance schedule is appropriate.

What to Expect During the Healing Period

After a deep cleaning, it is normal to experience some tenderness or sensitivity in the treated areas. The gums may also appear to look slightly different as swelling reduces and healthy tissue regenerates. These changes are a positive sign of healing.
